• 免疫接種紀錄(針卡)副本

• 證件相片

• 報名費繳費紀錄

3. 報名費：$40

4. 繳費方式：

• 自動櫃員機入數

• 網上銀行

• 轉數快

＊將繳費記錄上載及保存作日後核實
